There are growing options in this area, but one of the biggest is Smile Direct Club (SDC). This option allows you to skip going to an orthodontist. With this, there is no orthodontist office exam. Sounds good- maybe this option is for you?
At home treatment with SDC starts with impressions you take at home, and send into the company. The impressions need to be perfect, or expect delays in getting your aligners. But the steps to getting the aligners also involve a process that may result in you not being approved for treatment, especially if your case is too complicated. In other words, if you need more than 20 aligners to correct your teeth, you are not approved.
That being said, if
1. your case is “mild”, and
2. you’re not too picky,
3. you’re looking to spend as little as possible,
4. you’ve informed yourself,
Then direct to consumer may be an option for you.
But for most patients, straightening teeth is complex. All through the year, we answer questions in the office about teeth whitening. It’s important to remember that what we do everyday has an impact on the color of our teeth, so
- Always remember to brush 2 minutes twice per day and floss daily
- Avoid dark beverages, or brush whenever you can after drinking them (this includes sodas, sports drinks, wines, etc.)
A recent meta analysis of home-applied teeth whitening products, similar to those we have available for our Dover, Smyrna, and Kent county patients, showed that 73% of people who whiten their teeth will experience a whitening that is 2 shades lighter than the placebo group. 20% of people will experience a whitening effect that is 5 shades lighter. The CDC estimates the prevalence of ASD (Autism Spectrum Disorder) is one in 88 live births (one in 56 boys and one in 256 girls). These patients are at greater risk for oral problems, due to behavior and communication issues, personal neglect, avoidance of social contact, and medication side effects.
A recent article* (reference available on request) discussed applying the Applied Behavioral Analysis (ABA) – a learning method used to help these patients build “socially useful repertoires”, and “reduce socially problematic ones.”
One way to implement this method is by using a Picture Activity Schedule – a series of pictures showing the procedures to learn.
A study to test the effectiveness of this method was carried out at Nassau Suffolk Services for Autism. Each child’s baseline results were compared with his or her post-ABA results. Patients were shown pictures of the planned procedures twice a day for 2 weeks.
After 2 weeks of ABA training, 13 completed all steps (more than before), 2 completed some and the same student failed to complete any of the steps. The students also showed significantly faster completion times after picture training.
At OSL, we have also implemented this concept, and we are thrilled with outcomes to-date:
1. We use videos demonstrating the planned procedure with all of our patients as a way to prepare for a successful appointment, or
2. We ask the non-ASD sibling to go first for the records with the ASD sibling watching his or her sibling
3. We show the patient a picture of what the next step is to communicate to the patient in the chair.
Our orthodontist, Dr. Stephanie Steckel agrees with the American Association of Orthodontists’ (AAO) recommendation that a child should receive his or her first orthodontic evaluation by the age of 7. By the time a child reaches this age, several permanent teeth have erupted, allowing orthodontists to identify possible problems. For some young children, early treatment can reduce the time spent in braces and possibly eliminate the need for future tooth extractions. Even though many children do not need treatment when they are young, a thorough evaluation can help determine if treatment would be beneficial for the child’s dental future.
When Dr. Steckel has the opportunity to see patients at an early age, she can correct small problems before they become larger, more expensive ones. Early orthodontic intervention may consist of the patient wearing removable or fixed appliances to allow for proper jaw growth and improved tooth spacing.

While starting orthodontic treatment at a young age can be very beneficial in many ways, it is never too late to achieve a healthy, straight and beautiful smile! These days, braces aren’t just for kids, and many of the patients we see at our Dover orthodontic practice are adults. Adults now have many choices when it comes to wearing braces. We offer several different types of braces including metal braces, clear ceramic braces, In-Ovation self-ligating braces and Invisalign, which is a very discreet way to improve one’s smile. Many of the adults we treat opt for clear braces or Invisalign as both are very low-profile in nature.
