MYTH: Patients LOVE their retainers

FACT: Patients are happier with straight teeth

Retainers are planned for our patients to reduce the the chance of relapse after orthodontic treatment. Alpine & Rafetto Orthodontics (ARO) – formerly OSL- provide retainers based on the doctors’ recommendations, and original diagnosis of the case. Our patients like their teeth to stay as straight as possible. Stability post-treatment is based on 4 factors: periodontal, occlusal, soft-tissue, and growth.*

Fixed retainers: Maintain your result better, and are difficult to lose.

We review instructions on wear and care the day the retainers are placed. Our ARO-OSL doctors lecture to local hygienists so they know how to help you keep your retainers intact as long as possible.

Removable retainers: Are made to fit well and we instruct on proper wear and care. This retainer can be lost at school, at a friend’s house, a restaurant… We go over ways to avoid this!

One study showed 86% of patients will experience various amounts of relapse if they do not wear retainers.

So LOVE your retainer, wear and care for it, and enjoy your orthodontic results!

MYTH: The Best Time to Begin Orthodontic Treatment is the Same for Everybody…

FACT: Dr. Steckel and the American Association of Orthodontists’ (AAO) recommend that a child should receive his or her first orthodontic evaluation by age 7 years. For some young children, NOT ALL, early orthodontic treatment can reduce the time spent in braces and possibly eliminate the need for future tooth extractions. A thorough evaluation can help determine if treatment would be beneficial for the child now or when to start in the future. These days, braces aren’t just for kids, and Invisalign isn’t just for adults!

MYTH: Orthodontists run behind schedule and keep you waiting for your appointment.

FACT:  There can be orthodontic emergencies that change and slow the schedule. However, those should be few and far between.

If you see pink…go to the sink!

Whether you use a power or manual toothbrush or interdental brush; rubber tip or dental floss, or water pik; mouthrinse, each of these oral hygiene products have instructions to follow for the best results.

Optimum Results=Plaque-free teeth, non-bleeding gums, and fresh breath
Poor Results=Plaque and food particles left on the teeth and surface of your tongue. The bacteria in your mouth then break down the food particles, releasing chemicals that have a strong odor leading to bad breath.
One way to check that your oral hygiene efforts are working is to use a pink disclosing tablet after brushing and flossing. This tablet produces a harmless pink stain that clings to plaque, the film of bacteria that is always forming on the teeth and gums. It is a built in check for the user to see where he/she missed brushing.  If you see pink…go to the sink, and brush off the plaque to reveal your clean teeth.
If Dr. Steckel recommends a mouth rinse for you, then it’s best used after your teeth are completely clean.

“I have 2 rows of teeth”

Baby teeth are great space holders for our permanent teeth… until they are not!
We generally lose all of our baby teeth by age 12 years of age.
If they are not lost on schedule, eruption problems with our permanent teeth can occur.
Dr. Steckel sees patients starting at 7years of age and older to evaluate growth, jaw patterns, function, baby teeth, and esthetics to properly time the start of orthodontic treatment.
The patient below  was 15 years old at his first appointment and he had 2 baby teeth.
The baby teeth blocked the permanent teeth and forced them to erupt on his palate. The baby teeth were removed and then the patient started Teen Invisalign therapy. He wore his aligners and finished with all permanent teeth aligned, and a
great smile!

Make Space, Reduce Treatment Time and Keep Your Teeth!

Orthodontic treatment enhances the attractiveness of your smile, and one way Dr. Steckel can create space for your teeth to straighten is using IPR (=Interproximal Reduction) to “slenderize” your teeth. This procedure also reduces the development of “black triangles” between the teeth-which happens commonly with adult patients.
A research study was carried out to assess dental health after IPR. 43 consecutively treated patients ages 19-71 years of age who had IPR done 4-6 years previously were examined for dental decay. None of them reported sensitivity to temperature after the procedure, and the difference in decay development between the treated (2.5%) and untreated teeth (2.4%) was not significant.*

DIY Plastic?

DIY PlasticsIf you Google “Straighten my teeth”, one search result is “direct to consumer.”

There are growing options in this area, but one of the biggest is Smile Direct Club (SDC). This option allows you to skip going to an orthodontist. With this, there is no orthodontist office exam. Sounds good- maybe this option is for you?

At home treatment with SDC starts with impressions you take at home, and send into the company. The impressions need to be perfect, or expect delays in getting your aligners. But the steps to getting the aligners also involve a process that may result in you not being approved for treatment, especially if your case is too complicated. In other words, if you need more than 20 aligners to correct your teeth, you are not approved.

That being said, if
1. your case is “mild”, and
2. you’re not too picky,
3. you’re looking to spend as little as possible,
4. you’ve informed yourself,
Then direct to consumer may be an option for you.

But for most patients, straightening teeth is complex. Achieving excellent results for my patients at Orthodontics on Silverlake (OSL) in Dover involves knowing you, your diagnosis, and any limitations in planning your treatment. Most patients getting aligner treatment need more than 20 aligners to correct their bite and address the concerns. I am an “ Elite doctor” with Invisalign and have successfully treated hundreds of patients since 1999. The vast majority of my patients love the Invisalign experience and the results.

DIY Orthodontics?

About half of adults use the internet to access information regarding their health decisions. However, not all of this information is accurate, or easy to understand. Some information has the potential to be dangerous. There are Youtube videos describing the use of rubber bands, paperclips and other household items for “Do it Yourself” (DIY) orthodontics. Many authors of orthodontic topics, such as oral hygiene with braces, or treatment with Invisalign are orthodontists. Dr. Steckel publishes this monthly blog on our website to provide accurate, easy to understand, and relevant answers to common questions.

Her sources for these blog topics are the current literature, and clinical experiences in her practice. Please see previous OSL blog posts on February 2015, and May 2014 as examples.

The concern with DIY is the risks of unintended consequences, and gum tissue not being healthy enough for orthodontic treatment. The risks include loss of teeth, damage to gum tissue, and the need for expensive restorative treatment to replace the damaged teeth.

You do not need a referral from your dentist to make an appointment with Dr. Steckel. At your first appointment, she will determine the health of your gums and teeth, and help you choose which orthodontic option will benefit you. Orthodontic treatment can transform your smile, and the foundation for this is healthy gums and good oral hygiene!

Answer: Invisalign is!

Advantages for Invisalign:
1. Easier brushing and flossing, less dietary restrictions, no “wire pokes”.
2. Comfortable to wear, and not obvious to most people that you have the aligners on
What patients need to know: It will change your daily routine to some extent. You need to wear them 22 hrs per day to complete treatment on schedule.
Disadvantages of Invisalign:
1. Being consistent with your aligner schedule is very important, or the treatment can take longer or not work.
2. Some patients still need braces on some of their teeth to get the best results. This can be determined at your initial appointment with Dr. Steckel.

“Brush up” on Your Knowledge for Children’s Dental Health Month

Children’s Dental Health Month is a great time to “brush up” on dental health knowledge. It is important to brush two times a day, using small circles, for two full minutes. Watch the video below to see how.
